The file and the job disagree more often than leaders want to admit. This sheet is how you find that out in seven minutes without announcing a safety visit.
Print it. Walk one job that is running now. Write the energy you can put a hand on. Write the control the paper claims. Write the gap. Then tell the supervisor before you leave.
A supervisor who owns a crew this shift. An EHS person who is tired of touring the pretty area. A plant lead who wants to know whether the permit matches the work. A superintendent who can spare seven minutes and not a two hour audit.

Fill the lines. Job. Date. Who is doing the work. Energy present. Control you can put a hand on. Control the paper claims. Gap. Who is in the line of fire. Who can stop this job, and whether they said so. What you told the supervisor. Owner of the next action. Date that action is due.
If the file and the job disagree, the file is wrong until the job changes. Do not fix the paper first. Fix the energy. Then write what is true.
It is not an inspection form for the whole plant. It is not a behavior observation card. It is not a monthly metric. It will not replace a real investigation. It will not make a bad system look good if you walk the show area.
One job. Seven minutes. That is the point.
